This is similar to 〜だろう (darou), but it’s more about deep, personal, and often past emotion. It’s rhetorical. You’re not asking a question, you’re sighing, “Man, how hard that was…” or “How many times I…”
You could use this when you’re reflecting on the hard times. “Man, how many times did I almost die to get here?” (ここまで来るのに、何度死にそうになったことか。) It’s not a question for anyone, it’s just a deep, emotional statement.
This is the “looking out the rainy window” grammar. You’re deep in your feelings, reflecting on the struggle. It’s the end of a long, hard-boiled movie when the detective thinks, “How much have I lost…”

This pattern is almost always used with a “how much” word like どんなに (donna ni), どれほど (dorehodo), or 何度 (nando). You take the plain form (past or present) of a verb or adjective, stick one of those “how much” words at the front, and end it all with ことか.

- この「シノギ」を成功させるのが、どれほど大変だったことか。 この 「シノギ」 を せいこう させる の が、どれほど たいへん だった ことか。 (kono “shinogi” o seikou saseru no ga, dorehodo taihen datta koto ka.) You have no idea how hard it was to make this hustle a success.
- 刑務所の中で、どれだけ自由を夢見たことか。 けいむしょ の なか で、どれだけ じゆう を ゆめみた ことか。 (keimusho no naka de, dore dake jiyuu o yumemita koto ka.) How much I dreamed of freedom when I was in prison.
- あいつが裏切ったと聞いた時、どんなに驚いたことか。 あいつ が うらぎった と きいた とき、どんな に おどろいた ことか。 (aitsu ga uragitta to kiita toki, donna ni odoroita koto ka.) When I heard he betrayed us, man, how shocked I was.
